Medical Visits

WHOLE BODY. WHOLE FAMILY. WHOLE HEALTH

My Cultivate Whole Health practice is an inclusive and welcoming environment for patients, regardless of race, gender, sexual orientation, or physical ability.”

Dr. Chandra has a solo part-time clinical practice and works with patients via telemedicine and in-person visits at the AIMS Institute in Seattle where she has office space.

Established Patients

New Patients

Dr. Chandra is presently seeing a limited number of new patients. As a solo part-time clinical practitioner, she has moved out of the private insurance model to provide the level of care that allows for the depth and breadth of service toward your health needs and goals, as insurance reimburses very little for this care.

It is recommended that patients have medical insurance and are required to be established with a primary care provider when establishing in our practice as she is no longer a primary care provider.

If you have a private insurance plan, including Medicare Advantage

Fees are paid at the time of service at each visit through the private and secure patient portal. An invoice showing “paid in full” is available immediately on the portal for you to submit to your insurance for out-of-network benefit coverage if that is something that your plan offers.

If you have Medicare as your primary plan (not Medicare Advantage or QMB):

Your visit will be billed at set Medicare rates, and you will be required to pay for the visit at the time of service through the private and secure portal. We will then send the Paid-in-Full Invoice to Medicare and they will reimburse you at about 70% of the paid amount in approximately 60 days.

With any of the above plans, common/conventional lab testing through Labcorp and Quest is billed by the lab to your insurance and is typically covered.

Depending on your concerns, she may order Functional Medicine laboratory testing such as a stool/digestive/microbiome analysis, salivary or urinary testing for hormones, toxins, mold; and genomic testing. Please note that these are often not covered by insurance. The price varies depending on the lab. Functional Medicine labs not covered by insurance generally range from $85-$400 each.

With this approach, we typically utilized nutrients — through food, and through nutraceuticals (nutritional supplements) to help restore optimal functioning of biochemical pathways. Please note that these are not covered by insurance, unless through an HSA/FSA plan.

“I will always work to incorporate your budget with your health needs in diagnosis and treatment plans. Many of the nutritional supplements I recommend are offered at a 25% discount.”

Please visit our Patient Portal and click “New Patient Registration” to request a discovery call, review clinic policies and fees, or schedule a new patient appointment.
